Analysis of 100 aorto-coronary artery bypass surgery--especially about the graft patency

From Sept. 1977 to July 1981, a consecutive 100 cases of aorto-coronary saphenous vein bypass surgery were investigated as to prognosis, mortality rate, graft patency and hemodynamic status. Operative mortality rate was 4% and total mortality rate was 5%. Prognosis was good in the 82% of these cases in their daily life. In 2 cases vasospasms were the cause of death. Only 3 patients were readmitted to a hospital with severe angina reattack. Acute myocardial infarction occurred in 2 patients at a late postoperative period but all were alive. Relation between the grafted coronary artery, vascular resistance and the graft blood flow was examined. This relation gave prognosis of the graft patency in the early postoperative stage as the following formula; y = ax and y = a'x. (y = graft flow; x = vascular resistance; a = 2/5, a' = 1/5). Graft patency could be preestimated as the following: y .gtoreq. 2/5x, excellent (patency rate was 97%); 2/5x > y .gtoreq. 1/5x;, good (patency rate was 92%); y < 1/5, poor (patency rate was 0%). Nitroglycerin, prostaglandin E1 and dipyridamole were studied as to effects on graft flow. Nitroglycerin and prostaglandin E1 decreased the graft flow and dipyridamole increased it. It was not useful to increase the graft flow with dipyridamole because of the bleeding tendency.
