Analysis of 34 severe, interstitial cases of pneumonia in children in the course of acute leukemia (AL) is presented. Pneumonia developed in the period of an intensive chemo- and X-ray therapy already during hematologic remission. As etiologic factors, gram-negative bacteria were most commonly found, particularly Klebsiella pneumoniae. The possibility of a toxic and/or allergic reaction of the lung to treatment with cytostatics is discussed. Rapidly developing symptoms of circulatory and respiratory insufficiency with leukopenia below 1000/mm3 always indicated a bad prognosis. In 40% of cases pneumonia, despite intensive treatment, was a direct cause of death.
