Neurophysiological investigations (EEG, ERG ) were carried out in 7 patients (5 girls, 2 boys) with proven ophthalmoplegia plus. A variable excess of irregular slow activity with patchy distribution was seen in the EEG of all cases. In 3 patients, additional abnormal findings were observed: multifocal sharp elements; abnormal sensitivity to photic stimulation; and in 1 case, brief generalized paroxysmal features during sleep. The ERG was of usual amplitude and waveform in 3 of the 6 cases investigated, while poor or absent in the other 3 patients. The VEP (flash) was definitely abnormal in 2 cases. The neurophysiological findings suggest variable involvement of brain and visual system in this disorder.
