The physiologic and morphological effects of pulsatile and nonpulsatile extracorporeal circulation on the peripheral vascular circulation for 4 h in mongrel dogs were compared. The diposable pulsatile blood pump for extracorporeal circulation, NIKKISO PP -11 Type, has 2 soft leaflet valves and a U-shaped silastic blood chamber. Extracorporeal circulation with pulsatile flow offered the following significant advantages: higher renal blood flow, higher renal tissue blood flow, higher duodenal tissue blood flow, lower renal vascular resistance and lesser ultrastructural degeneration of kidney and pancreas. There was no significant difference in O2 consumption, central venous blood PO2 [O2 pressure], urine volume, urinary electrolytes (Na/K ratio) and osmolality (urine/serum ratio) between 2 groups. Apparently, pulsatile flow during long-term extracorporeal circulation offers significant advantages in terms of better peripheral circulation and prevention of degeneration in individual organs.
