Two patients with major chest trauma, significant flail chest and paradoxical respiration were treated successfully by immediate tracheostomy, intermittent positive pressure respiration (IPPR) and intercostal drainage. Complete healing of the thoracic cage with minimal alteration of pulmonary function was achieved in both cases by the end of 3 mo. Recent literature on the subject was reviewed.
