Low-calorie-formula diets--are they safe?

A low-calorie (320 kcal) formula diet was effective in 44 obese patients treated for 1-2 months. Substantial weight losses (mean=8.9 kg at 4 weeks, 11.0 kg at 8 weeks) were attained in all patients, and nitrogen balance equilibrated by 5-6 weeks. The diet contained carbohydrate, milk protein of high biological value, and adequate minerals and electrolytes. No ECG abnormalities were seen in any patient during the first 4 weeks of dieting; 1 patient exhibited an ECG change after the sixth week. Biochemical analyses indicated decreased serum cholesterol and increased bilirubin and alkaline phosphatase; these minor changes were not considered clinically significant. Hypertension medication had to be reduced in 1 patient. These findings demonstrate the safety and effectiveness of this formula diet as a treatment for obesity. However, cardiac monitoring and medical supervision are recommended for all patients on low-calorie diets for longer than a month. (nm)
