Effect of variability in ramification of the left branch of the bundle of His upon interventricular septum activation

The spread of excitation in the interventricular septum was simulated on a computer model consisting of a set of 5558 points occupying evenly 1/2 of a thick-walled hollow cone, oriented in space analogically to the real septum. The reconstruction of 49 normal variants of the ramification of the left branch of the bundle of His previously published Kulbertus served as the basis for determining the points of the initial excitation foci. The spatial vector, representing the border of the excited part of the model in the first eighth of the total simulation time, was oriented forward and into the left half of the spread of values in preparations which were relatively high at the base of the septum with situated endings of a subtle and short anterior bundle. No other associations were found between the orientation of the resulting vectors and morphologic types of ramification of the left bundle branch. The described variant of ramification of the left branch of the bundle of His apparently is recorded in the ECG as missing the Q wave in leads I, aVL, V5, V6.
