Therapeutic consequences of measure of inactivation index of isoniazid during antituberculous therapy

Isoniazid dosage regimen was adapted during antituberculous therapy, according to the inactivation index method described by Vivien, in 1223 tuberculous patients. Calculated therapeutic dosages range from 18-3.22 mg/kg per day for fast acetylators (n = 619) and from 3-1.5 mg/kg per day for slow acetylators (n = 604). Children (n = 86) are proportionally faster acetylators than adults but the dosage regimen ranges are equal. Significant variations of inactivation index are mainly found during the treatment with patients showing extreme values of metabolic activity, i.e., for fast acetylators (I < 0.40) and for slow acetylators (I > 1.00). Regular adjustment of the dosage regimen seems particularly important for these patients.
