A rational therapeutic approach using the intracarotid injection technique for the treatment of thromboembolic stroke is described in 35 patients. The modality consists of sequential intracarotid injection of heparin, saline, a solution of Dextran 40-20% Mannitol-vasodilator-glucose and steroid. Proposed mechanism of action of the various agents used in the treatment is described. Immediate and permanent reversal of neurologic deficit was observed in the group for a period of observation extending from 6 mo.-5 yr. No complications or short-term mortality was associated with the procedure. A decline of morbidity and mortality in the natural history of the disease was observed. Five later deaths were due to acute myocardial infarction and sudden death, stressing the correlation between carotid occlusive disease, myocardial infarction and sudden death.
