Assessment of haemostasis in fifteen uraemic patients subjected to chronic haemodialysis

Hemostasis was assessed in 15 uremic patients with chronic renal insufficiency subjected to hemodialysis for 6 h a day, 3 days a wk. Factor [F] V was under the normal range in 12 of the 15 subjects, no inhibitors being found. A striking increase of FVIII activity was found in all the patients, with a mean value of 348%; there was a slight increase in FIX activity as well. Antithrombin III was increased in 5 patients. No hyperfibrinolytic activity was found in any case. Deficient platelet aggregation appeared in 4 patients using collagen, ADP and epinephrine, in 1 patient with ADP and in 1 other case with collagen. Some patients with normal platelet aggregation to collagen showed also shortened aggregation times with collagen and with epinephrine, which is compatible with an increased endogenous platelet ADP activity. The slope of the 1st aggregation wave, which measures the aggregation velocity, was altered in 4 patients to a final ADP concentration of 1.3 .times. 10-5 M. None of the patients showed clinical symptoms of hemorrhagic diathesis during the treatment.
