Peripheral blood circulation and oxygen tension in the blood and tissues of patients with cardiac insufficiency

Using the method of 133Xe clearance registration, Astrup's micromethod and O2 polarography, 78 patients with ischemic heart disease complicated with circulatory insufficiency were examined to determine the interrelations between the changes of the O2 tension in the blood and tissues and the tissue circulation. The examinations have shown that, as the circulatory insufficiency progresses, the volumetric rate of the peripheral tissue circulation and the O2 tension in the blood and the tissues exhibit a tendency to decrease. These changes were of a phasic character; for each phase the specificity of the disturbances in the peripheral circulation mechanisms and in the processes of O2 exchange was determined. A dependence of the O2 tension in tissues on that in the blood is demonstrated. The changes of the volumetric rate of tissue circulation, disturbances of the vascular permeability and of the degree of O2 extraction and utilization by tissues, and practically complete incapacity of the peripheral circulation system to expansion at later stages of the circulatory insufficiency development lead to a qualitatively new state of tissue respiration in which anaerobic processes begin to prevail.
