Effects of glucagon (15 mg/h) on hemodynamics and myocardial metabolism were investigated in II patients early postoperatively following open-heart surgery. Patients were grouped according to the changes in coronary sinus PcsCO2 increased significantly (P .ltoreq. 0.05) compared to both before administration of glucagon and to Group II. Increase in cardiac index were observed only in Group I. As a reflection of improvement in hemodynamic and myocardial metabolism, arterial lactate, pyruvate and the decrease in arterial K+ as a result of increase in A-CS K+ (myocardial K+ uptake) were greater in Group I than in Group II. The inotropic action of glucagon is mainly mediated by the increase in arterial insulin level which enhances myocardial metabolism with resultant hemodynamic improvement.
