The postdialysis rise in the activity of such enzymes as SGOT was studied in 50 chronic hemodialysis patients. Changes in enzyme activity, expressed as percentage of predialysis activity, were assessed in 100 paired pre- and postdialysis serum samples and compared with the changes in serum total protein concentration and, in 10 patients, in plasma volume. The changes in SGPT, SLDH, total protein and plasma volume were comparable. The change in mean SGOT activity exceeded that in total protein as a result of a marked increase in SGOT activity in patients with low predialysis activity; the increase in SGOT in patients with normal predialysis activity did not exceed that in total protein. Changes in SGOT activity in patients dialysed without or with minimum ultrafiltration were also comparable to the changes in total protein. No dialysable enzyme inhibitor appears to exist; hemoconcentration may be a major cause of this phenomenon. The marked postdialysis change in SGOT activity in patients with low predialysis values suggests an additional laboratory artefact.
