Severe hyponatremia in congenital hypothyroidism

An infant is presented with congenital hypothyroidism associated with hyponatremia manifested by a low serum osmolality, an elevated urinary Na+ excretion and a high urinary specific gravity. Thyroid stimulating hormone, protein-bound iodine, triiodothyronine (T3) and thyroxine (T4) assays are used to diagnose congenital hypothyroidism. Inappropriate antidiuretic hormone secretion is suspected to cause water and electrolyte imbalances.
