Obstructive jaundice caused by blastomycosis of the lymph nodes around the common bile duct

A case is reported in a 23-yr-old man who presented with a 2-month history of abdominal pain and jaundice. Abdominal ultrasound and tomography revealed a distended gallbladder, dilatation of the intra- and extra-hepatic bile ducts with enlarged lymph nodes of the hepatic hilus. During surgery, these observations were confirmed and histopathology of lymph node biopsies showed Paracoccidioides brasiliensis. The patient was successfully treated with amphotericin B (total dose, 2 g over 60 d).
