Effect of a calcium antagonist, nifedipine, on the cold pressor test in normal subjects

Nifedipine, a nitrated 1,4-dihydropyridine derivative, antagonizes the increase in peripheral resistance and the resulting increase in arterial blood pressure induced by a cold pressure test (CPT) [the subject's hand is immersed in ice cold water for 2 min]. Nifedipine is useful for controlling angina pectoris and hypertension. Nifedipine has a Ca-antagonist effect which inhibits transmembrane flow of Ca ions in myocardial and smooth muscle during the excitation phase. Free Ca ions necessary for activation of myofibrillar ATPase and transforming phosphate-bound energy in mechanical work are in lesser supply due to the inhibitive effect of nifedipine; peripheral vascular resistance is reduced. Arterial pressure and the increased cardiac frequency induced by the cold pressor test are reduced. The ECG does not show material changes.
