The effect of hydration and dehydration on renal function in surgical patients

Renal function was assessed before, during and after light balanced general anesthesia for abdominal surgery in 8 patients receiving intraoperative volume expansion with lactated Ringer's solution. Four patients received no fluids from midnight until the time of surgery; the other 4 patients received hypotonic fluids at the rate of 125 ml/h from the evening before surgery. Urine flow was higher and urine osmolality was lower in the hydrated group. Renal plasma flow and glomerular filtration rate rose significantly (pH 0.05) and filtration fraction fell (pH 0.10) during the intraoperative period in the hydrated patients.
