No objective conclusions can be drawn regarding the most efficacious form of therapy for the postgastrectomy bezoar. There are several forms of therapy which have been employed successfully and it should be possible to avoid surgery in most cases. Oral cellulase enzyme is not being used as the initial intervention after roentgen diagnosis of a postgastrectomy bezoar because of the ease of administration and the high degree of success. Symptomatic and roentgenographic resolution may obviate the need for diagnostic endoscopy. Of the 4 cases, case 4 illustrated the simple outpatient management of this problem. If this approach is unsuccessful other approaches, including those which require gastric intubation, should be attempted.
