The effectiveness of cava-pulmonary anastomosis in 145 human patients with Fallot's tetrad and tricuspidal atresia was studied over a period of 17 yr. The leading determinant of the operation's success was the condition of the pulmonary-arterial bed. An increase in venal pressure and development of cava-to-cava fistulas were observed in patients with manifest arterial hypoxemia, narrow main branches of the pulmonary artery and low plasticity of the vessel bed resulting from increased pulmonary-arterial resistance. This was a leading cause of recurrence of arterial hypoxemia. In some patients progressive obliterating processes in the left pulmonary artery and increased pulmonary-arterial pressure followed the operation.
