The effect of thiazide diuretics on carbohydrate-induced calciuria in patients with recurrent renal calculi

Twenty-one patients with recurrent renal calculi were given 100 g glucose by mouth before and after 6 months of hydrochlorothiazide therapy. Glucose before therapy increased Ca and Mg excretion, Ca in urine and net acid excretion and decreased Na and K excretion. During therapy basal Ca excretion was lower, urine pH and ammonia higher and Na, K and net acid excretion unchanged. After glucose during therapy Ca excretion increased, but less than before thiazide therapy, and the Ca concentration far less, as urine flow tended to increase after glucose. Increases in Mg and net acid excretion were the same as before thiazide was given. Na excretion was unchanged and K excretion decreased less. The persisting calciuric effect of glucose after thiazide indicated that they affect different tubular sites or mechanisms of Ca reabsorption.
