Errors in insulin doses due to the design of insulin syringes

When insulins with different durations of activity are mixed in and administered from the same syringe, errors may occur. All syringes have space in the tip and in the needle hub. The volume of this dead-space depends upon the style of manufacture. When mixing most medications the dead-space is not significant, but when mixing medications of small volume, the error can be significant. Two examples of such errors in human diabetics are described.
