Takotsubo cardiomyopathy (TTC) is a rare but an increasingly well-recognized reversible cardiomyopathy that is frequently precipitated by a sudden emotionally or physically stressful event and has a clinical presentation that mimics a myocardial infarction (MI). TTC is characterized by transient, left ventricular systolic dysfunction in the apical portion and hyperkinesia in the basal segments with no evidence of obstructive coronary artery disease. TTC has been reported in the elderly, in English literature, but it is rare in advanced age. This is a case presentation of a 95-year-old patient who presented with severe epigastric pain and tachycardia, initially diagnosed as a ST elevation myocardial infarction but with normal coronary angiography. This case can be a reminder that even in late-elderly people TCC should be considered in the differential diagnosis.
