The article reports on 4 Puerto Rican patients aged 32-63 years with the slow onset of spastic paraparesis without sensory deficits. Cranial nerves were unaffected. Full evaluation included myelogram-CT scans, and brain and cervical cord magnetic resonance imaging. A firm clinical diagnosis of primary lateral sclerosis was made. Three other spastic paraparetic patients, 2 with HTLV-I infection, and 1 with multiple sclerosis were included for diagnostic comparison. A protocol for investigating patients with chronic spastic paraplegia is provided.E.M. Scrimgeour.
