Spastic paraparesis due to cryptococcal osteomyelitis. A case report

A case in a 50-yr-old coal miner is reported. Radiography revealed large cystic changes in the 9th, 10th and 11th thoracic vertebrae. Bone scintigram with 99mTc pyrophosphate showed increased uptake by these 3 vertebrae. Cryptococcus neoformans was identified in necrotic and granulomatous tissue removed surgically. The patient was successfully treated with 2 decompression operations, amphotericin B (total dose 1105 mg) and 5-fluorocytosine (total dose 2202 g).
