Private practice nurses, in partnership with the general practitioner, provide the majority of home care and primary care of elderly people. They are therefore best placed to prevent and detect decompensation risk factors in their frail elderly patients. A pilot study aims to assess the impact of a tool on the quality of transmission of information with the general practitioner and of the global care management.
