Preoperative neo-adjuvant combination hormone therapy induces a number of characteristic changes that were already reported; however, we found that after hormone therapy tumors tend to have a smaller volume and lower incidence of capsular and surgical margin invasion (i.e. down-staging of the disease). There is also a striking lower occurrence of prostatic intraepithelial neoplasia. There is also a higher Gleason score, which we think is probably artefactual due to changes induced by the therapy, and smaller nucleoli were also found. In terms of prognosis for the patient, the effect of these changes will have to be evaluated further in the future.
