Cutaneous sensitivity to C. immitis. Comparison between the spherulin and coccidiodin reaction in man

Spherulin prepared from the spherule form of Coccidioides immitis was compared with coccidioidin in skin tests. Of 243 residents of an area in Mexico where the disease is endemic, 64% reacted to spherulin (2.8 mu g) and 41% to coccidioidin (1:00; 26 mu g). All reactors to coccidioidin reacted to spherulin. In 35% of the reactors to spherulin there was no reaction to coccidioidin. Spherulin provoked a strong reaction in a subject with disseminated coccidioidomycosis who did not react to coccidioidin. No evidence of false positive reactions to either reagent was found. Neither reagent provoked positive reactions in 86 people who had never been in an area where the disease was endemic.
