The number of persons who survive the initial injury of C-4 on -5 fracture has increased. As the wrist extensors are absent at this level, prehension is dependent upon an outside power source. CO2 and electric motor and battery packs have been used to power finger-driven splints and provide active prehension. There was a high rate of rejection of these by patients after discharge. Several reasons for rejection are given. A "ratchet" splint which provides an adjustable, static prehension is described and key points illustrated.
