Clinical investigation on diagnostic value of interferon-gamma, interleukin-12 and adenosine deaminase isoenzyme for tuberculous pleurisy

Pleural effusion specimens were collected from 190 patients seen at several hospitals in Beijing, China during March 2002-February 2003. Of the patients, 141 had tuberculosis (TB) and 49 had lung malignancy. The activities of adenosine deaminase (ADA) and ADA isoenzyme (ADA2) in the TB group were 51.6+or-10.9 and 47.9+or-6.9 U/litre, respectively. The levels were significantly higher than those in the malignancy group (P<0.01). Interferon (IFN)- gamma levels were higher in effusion samples from patients with TB than those from patients with malignancy (112.1+or-45.8 pg/ml vs. 24+or-5.9 pg/ml; P<0.01). Interleukin (IL) 12 concentrations were higher in samples from patients with TB than in those from patients with malignancy (104.3+or-32.3 pg/ml vs. 1.8+or-10.8 pg/ml; P<0.05). The sensitivity, specificity and validity of IFN- gamma and ADA2 in combination for the diagnosis of tuberculous pleurisy were 95%, 93.5% and 94.1%, respectively; corresponding values for IFN- gamma and IL-12 in combination were 90.1%, 92.2% and 91.2%, respectively.
