Incidental intrarenal-pelvic hydatid cyst discovered during percutaneous Nephrolithotomy (PCNL)

A 32-year-old woman presented with left lumbar pain and haematuria for 6 months [Pakistan]. Urine culture showed growth of Proteus mirabilis. Intravenous urogram and ultrasound of the kidneys showed a 3-cm radio opaque calculus in the left renal pelvis with multiple small daughter calculi. Evidence of partial obstruction leading to moderate hydronephrosis was also observed. After treating her urinary tract infection, she underwent percutaneous nephrolithotomy for renal stones. At surgery, multiple membranous structures embedded in gelatinous material along with small mushy stones and debris were observed and renal echinococcosis was suspected. Histopathology revealed extensively calcified cystic tissue, lamellations and focal necrosis of surrounding tissue. Echinococcal antibody titres were <1:16. She was treated with albendazole and abdominal ultrasound after 10 months showed no recurrence of the cyst.
