Isolated nocardial subcapsular and perinephric abscess

A 60-year-old diabetic woman presented with fever, abdominal distension, and tenderness of the abdominal region and left renal angle [Chandigarh, India; date not given]. There was evidence of large fibre sensory motor neuropathy in both lower limbs. Computed tomography scan of the abdomen showed a large subcapsular and perinephric abscess involving the left kidney. Aspiration of the abscess showed reddish brown pus, which upon staining revealed numerous branching acid fast bacilli resembling Nocardia sp. The patient was then treated with oral co-trimoxazole for 12 weeks, and had a good clinical outcome. It is concluded that timely diagnosis and appropriate treatment are essential to reduce morbidity and prevent mortality associated with the reported case.
