The case is described of a 39-yr-old white man presenting with a cough and haemoptysis. X-ray of the chest revealed an infiltration in the apex of the left lung. Thoracotomy revealed a necrotic lesion considered to be non-resectable and histological examination of the tissue was negative for acid-fast organisms and fungi. On the 9th postoperative day the patient died and histological examination of the left lung revealed acid-fast branching filaments compatible with Nocardia.
