Disseminated coccidioidomycosis discovered during routine skin cancer screening

An erythematous papule was noted on the left upper back of a 78-year-old man who presented for a routine dermatological examination [USA]. A biopsy revealed a large spherule with granular cytoplasm within a giant cell. The condition was diagnosed as disseminated coccidioidomycosis. Treatment consisted of oral fluconazole.
