Fungal pyomyositis in a diabetic patient as manifestation of myeloperoxidase deficit

A case is reported in an 18-year-old diabetic woman from Spain , who presented with a 15-day history of pain in the legs. Examination of the leg muscles revealed macular lesions and painful swellings. Studies revealed leukocytosis. Magnetic resonance imaging of the leg muscles showed abscesses indicative of pyomyositis. Candida albicans was cultured from fluid drained from the abscesses. Symptoms resolved after treatment with fluconazole for 6 weeks. 20 days after the end of therapy abscesses in the leg muscles reappeared and cultures again grew C. albicans. Further clinical examination revealed myeloperoxidase deficiency.
