An uncommon cause of high serum protein-bound iodine

Children with high serum protein-bound I (PBI) were 7 allergic to milk protein, found among children chosen at random for thyroid function tests, and one in hospital with chronic diarrhoea. All had been given a locally made [Israeli] soya bean substitute for milk. The 8 children were 2 to 7.5 months old at the start and their PBI was from 9.1 to more than 10 mu g/100 ml. One had a slightly enlarged thyroid gland and none had signs of hypothyroidism. In a 2nd blood test for 6 of the children, PBI had fallen to normal, 6.2 to 8.0 mu g/100 ml. By then the children had been getting smaller amounts of the soya feed, as they began to eat other foods, and 4 of the 6 had had no soya feed for 3 to 8 months.Soya feeds are normally fortified with I to counteract the goitrogenic effect of soya; well-known ones such as Mull-Soy or Soybean have 34 to 160 mu g I/litre but the local one had 1400 mu g/litre and it was calculated that the 8 children had been given from 445 to 2107 mu g I daily.
