Complete gynecologic examination can be accomplished in most children without anesthesia or sedation if kindness, tact, and the gentle touch are exercised. Some may need analgesia and only a few will need anesthesia. The examination is divided into four steps : 1) Inspection and palpation of the vulva - is every structure present ? Is it normal ? Will it function properly ? 2) Recto-abdominal examination to identify abnormal masses. Is Cx and uterus present ? 3) Examination of extra-pelvic sex areas - Breasts, Axillary hair, skin for acne, facial hirsutism. 4) Vaginoscopy and visualization of the cervix - in some cases. This requires small instruments. Most often errors in diagnosis result simply because complete examination is not carried out and lesions are thus missed.
