This review discusses the pathogenesis of and the effect of antiviral therapy on mother-to-infant transmission of human immunodeficiency virus type 1 (HIV-1) infection during antepartum, intrapartum and postpartum periods in breastfeeding and non-breastfeeding populations. The association between the mode of child delivery and HIV-1 transmission rates is described. The safety and toxicity of taking antiviral agents during pregnancy, the possibility of antiretroviral resistance, implementations of antiretroviral interventions, and strategies for implementation and future goals are also discussed. Ethical considerations regarding the prevention of mother-to-child transmission of HIV-1 are given.
