Wound healing disorders and their enzymatic therapy--a multicenter double-blind study

Three methods of topical enzymatic wound cleansing (collagenase/chloramphenicol=I, fibrinolysin/DNase=F, concentrated collagenase=lin) were tested in a total of 119 patients with wound healing impairment (above all ulcers of the lower leg, decubital ulcers, traumatic lesions) in double-blind .fashion within the scope of an efficacy and tolerability study. The documentation covered at least 14 treatment days with three intermediate controls and ended on day 20, i. e. after termination of treatment. The median duration of treatment was 9 days for 1 and lin and>20 days for F. Significant differences in favor of Im/I against F were found for complete wound cleansing and reduction in wound size. F was shown to be inferior to the two collagenase products as regards the favorable influence on the state of the edges and floor of the wound. The overall clinical assessment yielded a positive result for Inn in 91 %, for I in 78 and for F in 33 %. A similar picture was obtained for the tolerability of the three topical agents. The treatment had to be discontinued prematurely in two patients each under I and 1m and five patients under F because of side effects or lack of efficacy. An allergy developed in three patients under F. As regards the efficacy and tolerability, lin and I did not differ significantly in any of the parameters tested; however, both collagenase forms differed significantly from F (p<0,05). Collagenolytic topical agents can therefore be considered as being superior to unspecific proleolylic wound cleansing methods in wound healing.
