In response to a letter by L. Birkholtz et al. [South African Medical Journal (1998) 88, 58-60] on the prevalence of mixed species and antifolate-resistant malaria infections in Mpumalanga, South Africa, the correspondents comment on the use of a poorly-defined and small sample of Plasmodium falciparum malaria infections in the pyrimethamine resistance analysis and highlight the need for ongoing population-based drug efficacy studies to inform rational malaria drug policy in South Africa.
