This report follows the communication presented to the Academy by the authors in 1973. Their study is based on 332 patients with large incisional hernias treated between 1970 and 1984. Observations made between 1973 and the present confirm that large reintegratable incisional hernias give rise to respiratory disorders corresponding to the new notions of flail abdomen, paradoxical abdominal respiration and 4-stage respiratory rhythm. But further study of these cases provides new data of considerable practical interest : classification of patients as a function of the effects of respiratory preparation and the paradoxical consequences of the incisional hernia on the respiratory disorders of obese patients.
