Nocturnal intragastric administration of phosphates in the treatment of familial hypophosphatemic rickets

The reported terial of nocturnal intragastric administration of phosphates were carried out in 13 children with familial hypophosphataemic rickets (FHR) aged 3 to 11 years. The group included 6 boys and 7 girls. A mixture of phosphate salts was administered over night intragastrically through a tube using a perfusion pump, maintaining oral administration of phosphates intermittently in daytime. At the same time 1-alpha-hydroxyvitamin D3 was given in daily doses of 1-5 micrograms. The biochemcial control included determination before the treatment and on the 7th, 21st and 42nd days of the serum levels of calcium, phosphorus, alkaline phosphatase and in some cases parathyroid hormone, and 24-hour urinary excretion of calcium and phosphorus. In the case of hypocalcaemia development the dose of the vitamin was increased and the doses of phosphates were reduced. The described method was effective in acceleration of the biochemical normalization in patients with FHR. A significant reduction was obtained in the activity of alkaline phosphatase from 963 .+-. 341 u. to 721 .+-. 186 i.u. The use of the method of nocturnal intragastric instillation of phosphates in children with familial hypophosphataemic rickets should be considered in selected clinical situations, e.g. at the beginning of the treatment or before, during and after orthopaedic operations.
