Hepatobiliary cystadenoma with mesenchymal stroma can be associated with other hepatobiliary anomalies, and has the potential for malignant transformation. A 35-year-old woman from the Indian Punjab presented with hepatobiliary cystadenoma with mesenchymal stroma which was initially treated as a hepatic hydatid cyst.
