A diet with Ca 700 and P 1200 mg daily was given for 3 days to 284 patients all of whom had recent history of radiopaque renal stones. On the third day 24-h urine was collected and blood was sampled. The percentage of filtered P reabsorbed by the kidney tubules (TRP) was below normal in 29 patients of whom 25 had renal stones and was normal in 238 patients of whom 190 had stones. Of the 17 who had TRP above normal, over 90%, 12 had stones; 8 of the 12 were women. Of those 12, 2 had infections of the urinary tract, 2 had bilateral renal tones, 4 had high Ca in urine at study or before, 1 had diabetes and 1 had had the parathyroid removed a year before the test. All had normal endogenous creatinine clearance and none had evidence of obstruction on intravenous pyelography. All were believed to have a deficiency in thyrocalcitonin production.
