The hypothesis proposed is that ischemic cell death is a metabolic event as evidenced by ischemic contracture. There is a protective role for glycolytic ATP and for some drugs. There must be enough coronary flow to deliver the glucose. It appears that provision of glucose and restoration of blood flow have complementary benefits. Such combinations need exploration in patients.
