The New York City Bureau of Tuberculosis Control of the Department of Health has recently issued a series of recommendations for HIV-positive patients with tuberculosis (TB) who also require treatment with protease inhibitors. Recommendations outlining therapies using rifamycins, i.e., rifampin or rifabutin, adjusting to rifamycin-containing regimens, and therapy durations under specific conditions are listed. The Centers for Disease Control and Prevention (CDC) have developed similar but more conservative therapy guidelines. It is noted that the regimens offered have not been formally tested since most trials of protease inhibitors excluded patients requiring rifampin or rifabutin. The advice is based on suggestions made by experts in the fields of HIV, TB, and drug metabolism.
