Intrathecal application of morphine. IV. Analgesic effect on the pain in the head

Five patients with severe chronic pain in the head consented to the procedure. Three were suffering from cranial basal bone tumor, maxillary cancer and migrating disseminated cranial polyneuritis and 2 patients had trigeminal neuralgia. Their ages ranged from 53-62 yr. At 5-8 min after intrathecal administration of 0.5 or 1.0 mg morphine hydrochloride, 3 patients noticed reduction of pain and the analgesic effects continued over 24 h. In 2 other cases, judgment of the exact onset and the duration of analgesic effects of intrathecally applied morphine was not easy, but certain analgesic effects were confirmed by the reduction of pain and requirement for daily doses of other analgesic drugs. The side effects of intrathecally applied morphine, e.g., psychoneurological reaction, nausea, vomiting, dysuria, vertigo, were seen in all cases. Apparently a part of the intrathecally applied morphine reached the brain through the CSF, relieved the pain in the head and produced the side effects.
