Pulmonary hydatid cyst an unusual presentation

A 7-year-old boy was admitted to hospital in Haryana State, India, with malnutrition and respiratory distress. A chest X-ray showed hydropneumothorax occupying the whole of the right hemithorax. Surgery revealed a large cystic mass measuring 12 x 10 cm and occupying most of the upper and middle part of the pleural cavity. Excision was difficult but successful, and follow-up at 1 month showed full expansion of the lung on the right side.
