A 54-year-old farmer presented in Trivandrum, India, with isolated splenomegaly. The clinical diagnosis was portal hypertension. After splenoportogram and repeated aspirations, a diagnosis of splenic abscess was made. At laparotomy the spleen was found to be infected with a hydatid cyst, which ruptured spontaneously.
