Esophageal tuberculosis revealed by massive hemorrhage

Oesophageal tuherculosis is a rare lesion and its presentation by a massive haeimetemesis quite exceptional. The case reported here involved the problem of an tesophageai ulcer which was unrecognised prior to the massive haemorrhage -which required emergency exploratory laparotomy. Bleeding from the cesophageal site was partially controlled using a Sengstaken tube, inserted Peroperatively. The persistence of bleeding let to direct thoracic exploration the next day, revealing several non-haemorrhagic ulcers of the .oesophageal mucosa in the context of tuberculous inediastinitis. There were three episodes of minor bacmorrhage during the post-operative period, without any possibility of specific treatment. A massive haemorrhage on the 15th day proved irreversible. The surgical therapeutic Possibilities of such a lesion, resistani to Pressure occlusion, the') to medical antimberculous treatment are discussed.
