Neuroretinitis: an unusual presentation of ocular toxoplasmosis

The case of a 22-year-old man in Texas, USA, with a 4-day history of painless paracentral vision loss is reported. Examination of the optic fundi revealed the characteristic picture of neuroretinitis with papilloedema, splinter haemorrhages and macular star. Cerebrospinal fluid examination revealed a mild lymphocytosis. Toxoplasma ELISA titres were positive with IgG levels of 1:410. Treatment with clindamycin, sulfonamides and prednisone led to a satisfactory recovery. This is thought to be the first report of ocular toxoplasmosis presenting clinically as neuroretinitis.
